Health Care Proxy

The New York Health Care Proxy Law allows you to appoint someone you trust. This may be a family member or a close friend. The person you choose will make health care decisions for you if you are ever unable to make decisions for yourself. By appointing this trusted person, sometimes referred to as a health care proxy, you can be certain your doctors and nurses will follow your health care wishes. Your health care proxy will make all medical decisions for you for a long as you are unable to make these decisions yourself.

The decisions made by your health care proxy must be followed by all hospitals, doctors and other health care providers, like nurses. You decide how much authority you want your health care proxy to have and for how long that person is appointed. You can use the health proxy form on the website to write down who you decide your health care proxy should be and what kinds of health care decisions that person can make for you.

If you want, you can bring your health care proxy form to your doctorís office at Phillips Ambulatory Care Center and the staff will help you fill it out.
